Built on Real Caregiving Experience

Britta’s background as a CNA and Scott’s volunteer and private caregiving experience gave us a firsthand view of the tremendous need for quality, reliable care.

A Mission That Became Personal

Helping a friend move between rehab centers, home, and assisted living revealed just how life-changing care decisions can be. It also showed us how much thoughtful support matters in those moments.

A Care Plan Built Around the Person, Not the Process

1. No-Cost In-Home Consultation

Before services begin, we complete a comprehensive consultation covering safety, cognition, pain, sleep, nutrition, and emotional well-being.

2. Customized Plan of Care

We use what we learn to create a personalized care plan that reflects current needs, preferences, and day-to-day realities.

3. 30-Day Wellness Checks

Every 30 days, we reassess and update the plan of care to make sure it remains timely, effective, and aligned with the client’s changing needs.

Family Connection

Keeping Families Involved, Informed, and Connected

Families play an important role in the care process, especially when a client needs extra advocacy. When appropriate, loved ones are included in the consultation process and kept informed about care plan updates.

We also use the Wellness Hub, a unique technology that works through the client’s TV to support secure video connection with family and other important people in a restricted, HIPAA-compliant environment.

It is one more way we help reduce isolation, loneliness, and disconnection while supporting the whole family, not just the individual receiving care.

Starting care should feel clear and reassuring. Our onboarding process is designed to reduce stress and help clients and families feel comfortable from day one.

Free Consultation

We meet in the home to understand needs, routines, goals, and any concerns.

We finalize the service agreement and establish a thoughtful start date and care plan.

Caregiver Matching

We select a caregiver who is best suited for the client’s personality, needs, and situation.

Warm Introduction

On the first day, we accompany the caregiver to ensure comfort, clarity, and confidence.

Consistency & Communication

How We Help Ensure Reliable, High-Quality Care

Care Completed Each Shift

Caregivers address the tasks outlined in the plan of care and document what was completed at every visit.

HIPAA-Compliant Documentation

Visit notes and observations are logged electronically through a secure app to support accountability and visibility.

Coordinator Oversight

Our Care Coordinator reviews notes, follows up on concerns, and helps make sure quality standards remain high.

Ongoing Plan Updates

If changes are needed, we communicate with the client and family and adjust the plan so care stays responsive and effective.
